Environmental Scan Activity Forms

The following forms including related activities were created to help consortia conduct a local environmental scan. Of the many approaches to conducting an environmental scan, each consortium can decide the ones best suited to its structure and communication frequency. They might include surveys, questionnaires, focus groups and/or open forums.
